OR if you would like, you may pay at the time you arrive on October 18. Registration will begin at 6 p.m.

I waive and release all rights and claim for damage against the organizers or anyone else associated with CVAS’s Harrowing Halloween Haunt, for any injuries suffered by me or my dog as a result of taking part in the day’s activities.  I give permission for such first aid as deemed necessary for my dog and the humans in my party prior to transport to a hospital for further treatment. 

I will be responsible for the conduct of my dog, keeping him/her leashed and cleaning up after my pet. My dog will not be in heat nor is my dog known to be aggressive towards people or other dogs.

I certify that my dog is up to date on vaccinations and has a current dog license.

I understand that the eggs I will be hunting contain EITHER dog buscuits OR candy corn. I accept responsibility for any ingestion of human food for pets or pet food for people.
